Pharmacy Card

If available, this page allows you to view and print a copy of your pharmacy card used to get medications prescribed for your work related injury. The card is specific to your claim and employer.

Note: Supervisors and managers can only access this feature for their own claims, not those of others.

Accessing the Pharmacy Card

Your pharmacy card is available by clicking:

• View Pharmacy Card from the Actions menu beside a claim on the My claims page
• View pharmacy card from the Actions section of the Claim page

Printing the Pharmacy Card

Click Print near the top of the page to print the page using your browser’s standard print feature.

Most browsers allow you to print to a local printer or save the pharmacy card to a .pdf file.
